PARENT/GUARDIAN WAIVER & INDEMNITY AGREEMENT
In consideration of our accepting your child/children for participation in the above named activity, you hereby, for yourselves, your heirs, executors, and administrators, waive and release any and all rights and claims for damages that you may have against Elim Gospel Church and its agents, employees, representatives, successors and assigns for any and all injuries suffered by your child/children that arise out of the above-named program sponsored by Elim Gospel Church, including in transit to and from the above-named program.
You warrant that you have the right to authorize the foregoing and do hereby agree to hold Elim Gospel Church harmless of and from any liability of whatever nature which may arise out of or result from participation.
For the consideration stated above, you further agree that in the event that your child/children or you should make any claim against Elim Gospel Church for damages arising out of the above-named activity, you will personally indemnify, defend and hold harmless Elim Gospel Church and its agents, employees, representatives, successors, and assigns against any and all damage occasioned thereby, including attorney fees.
